Adoption Completion Form

A two week trial period begins from the date of this contract. Should you feel this dachshund is not suitable during this time frame, you will contact DRA and arrange a mutually convenient time to return him/her together with all the relevant paperwork. The adoptee is responsible for all costs involved in returning the dachshund to DRA. If the dachshund is returned to DRA within 2 weeks of the adoption date a full refund will be given. After 2 weeks from the date of adoption, no refund will be given.

If the dog is injured or sick during or after the trial period he/she will receive appropriate veterinary care and I agree to pay the costs of such veterinary care. I acknowledge that DRA is not liable or responsible for any costs associated with injuries or health issues after the date of this agreement. I also understand that any illness that has been diagnosed and is being treated at the time of the adoption, will be brought to my attention. I will receive instructions and medication, if necessary, for treating this dachshund.
